1) The size of the icon depends on you. You can choose from 300x300, 500x500, and I personally use 475x475. You can honestly use any size as long as it's a perfect square and it's not too big or small. Mess around with the sizes until you find the one that fits best.

2) Open up your image and size it to fit the square.

3) Add a layer on top. Using the paint brush, set the color to white and size it to be kinda big. Then add random white dots around the image it's okay if it get's on the model/character we'll clean that up later.

4) Choose a pinkish red color (the color changes with the PSD I used) and set the brush size slightly smaller and place dots inside the white dots.

5) For the final dots, set the brush to an even smaller size and in the color white and dot on top of the pinkish red dots.

6) Use the eraser tool and set it to a small size and hardness 100. Carefully go on the model and erase any dots that got on her.

7) Add any PSDs if you want to and then you're completely done!
